本フォームは、お持ちのカードの特典リワードを変更するための専用フォームです。（例：ベーシックからプレミアム、またはプレミアムからベーシックへの変更）
なお、旧ベネフィットカードから新ベネフィットカードへの変更はできません。新カードは新たにお申込みが必要です。
This form is exclusively for changing the rewards program of your
current card (e.g., switching from Basic to Premium or Premium to
Basic).
Please note that changes from an old benefit card to a new benefit
card are not accepted. If you wish to have the new benefit card,
please submit a new application.
